Incident and near miss reporting

Tell us when something goes wrong, or nearly does, the same day.

Report every incident and near miss

Applies to: everyone at [site or company], including visitors and contractors.

What you must do
1. Make the area safe and get help for anyone hurt. In an emergency call [number].
2. Tell [supervisor or role] as soon as you can, and always before the end of your shift.
3. Report near misses too. Nobody is in trouble for reporting.
4. Help the person who looks into it by saying what you saw.
5. Do not move equipment or clean up the scene until you are told you may, unless that is needed to keep people safe.

Why: near misses are free warnings. We fix the cause before someone is hurt.
